Issues start out small, and over time they grow towards completeness (description, better subject, etc) based on the feedback provided via comments and via on and off list interactions. One issue shouldn't be worrisome. But if there are more that don't get enhanced explanation-wise, or even many similar issues but resolved and closed as fixed or implemented, over a longer period of time then it looks like more is at play. Maybe it is something innocent that can be easily cured, such as ignorance. But it might also an indication of in-crowd behaviour, that there is no need to inform the outside or worse. Mentors should have an eye for this and report and address it.
